New Starter: Self Declaration of Criminal Offences Basic DBS Check

Appointment to the post you have applied for is subject to satisfactory criminal record and background checks. The level of DBS Check applicable to your job role is ‘Basic’. This means you are being asked to disclose conditional cautions or convictions which are not yet ‘spent’ under the Rehabilitation of Offenders Act 1974.
The information you supply on this form will be considered and if you declare any criminal cautions or convictions, these may be discussed with you prior to a final decision being taken on your appointment. Please be aware that the company has a policy on the Recruitment of Ex-Offenders (available on request), and having a criminal record will not automatically prevent you from being employed by us. In making a decision, Your Trust will consider many factors relevant to the appointment.
Your Trust adheres to the DBS Code of Practice, which requires organisations to use the Disclosure information fairly, not to discriminate against candidates on the basis of unrelated criminal offences, to ensure data security, and to comply with recommendations for the retention of records. A copy of the DBS Code of Practice is available on request or can be accessed on the DBS Website.
